Under the influence of the "research-oriented clinical pharmacist training" philosophy advocated by Director Lin Houwen, after years of rigorous scientific exploration, the clinical pharmacist team led by Gu Zhichun took the lead in establishing the "Innovative System for Medication Therapy Management of Multiple Chronic Diseases," achieving a series of innovative outcomes. Through the establishment of chronic disease medication therapy management clinics, they independently developed medication education items with independent intellectual property rights that are easy to disseminate, and formulated service standards and standardized operating procedures for chronic disease pharmaceutical clinics; pioneered a medication therapy management model from the perspective of optimal decision-making, established the first domestic "Municipal and Yangtze River Delta Hospital Anticoagulation Pharmacist Alliance," and independently developed the "Ai Kang Ning" intelligent medication therapy decision-making and management system. Based on evidence-based research findings, the system achieves patient medication risk prediction, optimal regimen recommendation, individualized dose calculation, and full-process medication management, constructing a homogenized and standardized service system for medication therapy management of multiple chronic diseases, thereby enhancing the efficacy of medication therapy management. Meanwhile, based on evidence-based pharmacy methods, they mined over 300 RCT studies and more than 500 health insurance data sets, completing 20 systematic reviews of medication regimens for chronic disease patients; applying discrete event simulation technology and fuzzy neural network deep learning technology, they successfully predicted the medication risks of narrow therapeutic window drugs such as warfarin in chronic disease patients, and applied a "balanced data strategy" to optimize model performance, providing medication dose recommendations for patients. The rational drug use system constructed based on evidence-based pharmacy promotes the updating of guideline evidence, changes clinical medication recommendations, and enhances the level of evidence-based medication use. This system has been successfully applied in multiple medical institutions across the country, achieving intelligent and homogenized chronic disease medication therapy management through tertiary-secondary-community hospital linkage, benefiting tens of thousands of chronic disease patients, significantly improving their medication awareness, satisfaction, and adherence, and realizing the transformation of medication therapy management from traditional manual and fragmented approaches to intelligent, full-process, and information-based approaches, significantly enhancing the level of rational drug use management in China. Based on this system, over 100 research achievements have been published, 20 of which have been cited by medical guidelines in North America, Asia-Pacific, and China, and it has received the Gold Award and Silver Award in the Discipline Management Group of the "China Hospital Management Award," and the First Prize of the Science and Technology Progress Award of the Ministry of Education (ranked second).
Gu Zhichun has presided over and participated in more than 10 national and provincial-level research projects focusing on anticoagulation evidence and anticoagulation management, and has published over 100 SCI papers as first or corresponding author in authoritative journals in the field such as eClinicalMedicine, Clin. Gastroenterol. Hepatol., Int J Surg, and Pharmacol. Res., and has been appointed as Associate Editor of a special issue of Frontiers in Pharmacology.
