The Department of Blood Transfusion was formerly the blood bank specialty group under the Department of Laboratory Medicine. In order to strengthen the management of clinical blood use, it officially became an independently established Department of Blood Transfusion in 2007. Over the years, under the leadership of successive department directors Professor Zhao Jinqiu, Professor Han Jieying, and others, the Department of Blood Transfusion has developed into a first-level clinical service department integrating pre-transfusion immunological testing and diagnosis, clinical transfusion supervision and guidance, medical blood reserve and management, and blood therapy, with comprehensive development in medical care, teaching, research, and management. Since 2013, under the leadership of the current Director Wu Jiang, the Department of Blood Transfusion has adhered to the principles of "building a solid foundation, improving capabilities, serving the clinic, and ensuring safety," taking the implementation of standards and norms as the guide, aiming to meet clinical needs, and strictly carrying out the work motto of "guarding transfusion safety as we guard life." At present, the Department of Blood Transfusion has 22 medical technical personnel, including 1 associate chief physician and 1 attending physician, 4 supervising technicians, and 16 technicians. In recent years, the culture of self-study and self-improvement has become common in the department. More than 90% of staff have a bachelor's degree or above, and 27.3% have a master's degree, changing the previous situation in which junior college education was predominant. The average age of the whole department is 36 years, full of vigor, vitality, and drive. The total medical area of the Department of Blood Transfusion exceeds 600 square meters, with complete facilities and a reasonable configuration, and it possesses a number of advanced technical devices, including the high-throughput fully automated blood grouping and crossmatching analysis systems represented by Shanghai's first "Erytra" and "IH1000." The Department of Blood Transfusion routinely carries out clinical transfusion diagnosis and treatment and rapid coagulation testing services. It can provide frontline blood grouping and coagulation diagnosis and blood product supply services 24 hours a day. It has a complete emergency support plan and transfusion green channel measures to ensure the timeliness and effectiveness of blood use for emergency and rescue. The transfusion compatibility tests currently carried out include multi-method ABO forward and reverse typing and RhD blood group identification, crossmatching, antibody screening and antibody identification, blood group reference and difficult crossmatching, prenatal diagnosis and prevention of hemolytic disease of the newborn, investigation of hemolytic disease of the newborn and adverse transfusion reactions, platelet antibody screening and platelet matching, etc. Rapid coagulation testing includes thromboelastography TEG, platelet aggregation and adhesion function, etc. Transfusion therapy includes: leukocyte depletion of various blood components, clinical transfusion consultation and guidance, discussion of difficult cases and case evaluation, peripheral blood stem cell collection and removal of pathological blood components, plasma exchange, and preoperative autologous blood transfusion, etc. Its business is comprehensive and its coverage is broad. The hospital's component transfusion rate reaches 99.9%, ranking among the leading levels in China; the pre-transfusion blood group serology immunology laboratory has continuously participated in the external quality assessment activities of the National Center for Clinical Laboratories of the Ministry of Health and has achieved full marks each time; the department supplies more than 50,000 units of various blood components for clinical use annually and has always maintained zero errors and no accidents; it has basically achieved independent completion of difficult blood group analysis and difficult crossmatching services; it actively assists clinicians in diagnosing and treating coagulation abnormalities such as coagulation disorders, microvascular thrombosis, and thrombocytopenia, achieving good results. In addition to completing a large amount of business work, the Department of Blood Transfusion also assists the Medical Affairs Office in undertaking the responsibilities of clinical blood use management and quality monitoring throughout the hospital, establishing a quality system and evaluation standards, and improving rules, regulations, and management measures. At the same time, it pays attention to the frontiers of disciplinary development, takes the lead in advocating the new concept of patient-centered patient blood management (PBM), and continuously expands its scope of business and improves the level of disciplinary construction. It participates in undertaking the Shanghai Public Health Key Discipline Construction Plan. Its main research directions include "establishment of methods and standards for evaluating transfusion efficacy," "evaluation standards and countermeasures for ineffective red blood cell transfusion," "basic and clinical research on transfusion-related immunomodulation and TRALI," and "transfusion preparation and efficacy evaluation for transplant patients," etc. As an expert committee member unit of the Shanghai Clinical Transfusion Quality Control Center, it assisted in drafting a number of construction standards for clinical transfusion departments and quality control inspection standards, and ranks among the top in routine quality control inspections.
